What Information We Collect

Information required by the Company to provide and deliver its products and services and products to you. This information may include, but not be limited to, name, address, email, phone, facsimile, employer, job function, business interests, and financial information.

Information that you provide to us for the purpose of registering with us, including for your warranty should you purchase our products, if applicable

Information that you provide to us in connection with our Support Center (that may include the use of a Chatbot, Returns, or Exchanges)

Information that you provide to us if you seek employment or seek to become an affiliate through the Website

 

Information about your visits to and use of this Website, and any other information you provide to us via the Website or by sending us an email or calling us

We do NOT want or seek to collect any “Protected Health Information” or any personal or patient-specific information concerning any individuals, patients, or other end users. The Company does NOT want or seek to collect any information or materials that would be covered by or subject to the Health Insurance Portability and Accountability Act (“HIPAA”) or the Health Information Technology for Economic and Clinical Health Act (“HITECH”). Therefore, in submitting any information to the Company or through the Application, you may NOT:

 

  • Submit any of your, or any third-party individual’s, “Protected Health Information,” which includes demographic information, medical histories, test or laboratory results, mental health conditions, insurance information, or other personal data that a healthcare professional collects or may collect to identify an individual and determine appropriate care
     

  • Submit any other information which may be, or may be used to specifically identify, any Protected Health Information of an individual. For example, you may be able to seek information on the Application concerning a particular medical condition, but you should not ever state whether you or any other identified individual suffers from or does not suffer from such medical condition.

 

Even if you do not submit personal information through the Website, we may gather navigational information about where visitors go on the Website and information about the technical efficiencies of our Website and services (ie, time to connect to the Website, time to download pages, etc). Collecting this information allows us to see which areas of our Website are most visited and helps us better understand the user experience. Additionally, collecting this information helps us improve the quality of the Website by recognizing and delivering more of the features, areas, and services our visitors prefer. We also may use “pixel tags” (also called “action pixels,” “Web beacons,” or “clear GIFs”) and/or JavaScript plugins, placed on our Website and in our emails to you. These Web beacons and plugins are small graphic images (typically that you cannot see) or code on a website or in an email message which are used for such things as recording Web pages and advertisements clicked on by a user, or for tracking the performance of email marketing campaigns. Privacy of Children Who Visit the Website

We recognize the importance of children’s safety and privacy. The Website is not designed to attract children and is not intended for use by any children under the age of 18. We do not request, or knowingly collect, any personally identifiable information from children under the age of 18.

Visitors From California

We collect various types of personal information about you during the course of your relationship with us. Based on the current limitations of the law, QC-Health is not subject to it. If you are a resident of California, please be advised of this and do not use this Website if this is a problem for you.

 

Visitors to the Website From Outside of the United States

If you are visiting the Website from a location outside of the United States, you should not use this Website and you are not permitted to do so. Your connection may be through and to servers located in the United States. All financial transactions on the QC-Health.org Website are conducted through the PayPal, Inc. payment gateway. By opening and using a PayPal account, you agree to comply with all of the terms and conditions, including privacy provisions, in the PayPal user agreement, located at www.paypal.com.

 

Governing Law

This privacy policy is governed by the laws of the State of New Jersey, without regard to its choice of law rule and without regard to the location of any user. Any dispute arising in connection with this policy must be filed in the state or federal courts located in New Jersey.
